What to expect: Your intake appointment is a chance for you to learn about Quest CHC, as well as for us to gather information about your health history and current well-being. At this meeting you can expect to hear about our programs and services as well as have any of your questions answered. Your second appointment will be with a primary care provider (doctor, nurse practitioner, or nurse) involving a health examine, as well as, addressing any health concerns you may have.
